Short Term Disability
- Weekly benefit: 60% of your employment income up to $1,500 per week
- Earnings definition: Your covered base salary, excluding bonuses and commissions.
- Definition of disability: You will receive benefit payments while you are “unable to work in your own job.”
- Duration of benefits: Short-term disability covers you from Day 1 for accidents and from Day 8 for illness related disabilities. You will receive benefit payments for 13 weeks while you remain disabled. After 13 weeks (or 90 days), your disability claim will be evaluated against and converted into a long-term claim if it meets long-term claim guidelines.
Long Term Disability
- Weekly benefit: 60% of your employment income up to $8,000 per month
- Earnings definition: Your covered base salary, excluding bonuses and commissions.
- Definition of disability: For the first 24 months of disability, you will receive benefit payments while you are unable to work in your own occupation. After 24 months, you will continue to receive benefits if you cannot work in any occupation based on training, experience and education.
- Duration of benefits: Long-term disability begins after you have been unable to work for 90 days due to your disability. Any valid long-term disability claims will be payable through social security normal retirement age (SSNRA). Should death occur during a long-term disability, up to 3 months of survivor benefit will be paid out to beneficiaries.
- Integration with other benefits: Long-term disability payments will be reduced by any Social Security disability benefits that you or your family members are eligible to receive; benefits will also be reduced by other forms of income you receive, such as Worker's Compensation and sick leave.
Life Insurance and Accidental Death & Dismemberment (AD&D)
- Benefit amount: One-time annual salary up to $150,000
- Accelerated life benefit: If you are diagnosed with a terminal condition, you can receive up to 50% of the benefit amount in a lump sum accelerated payout.
